1974年の多摩川水害が背景にある。この水害で多摩川の堤防が決壊して19棟の家屋が崩壊・流出したが、家を失ったことのほかに家族のアルバムを失ったことが大変ショックであったという被災者の話を脚本の山田太一が聞き、そこから作品の構想が生まれた。
ラストの水害で家が流されるシーンは、実際の報道映像が使用されている。

主演の八千草薫は、和泉多摩川駅の向かいのホームに佇む美しさに惹かれたといって電話をかけてきた竹脇無我と家族に隠れて不倫する主婦を演じ、それまでの良妻賢母的なイメージを打ち破り、新たな役どころを開拓。
関川夏央は「貞淑を絵にかいたような八千草とラブホテルの組み合わせは衝撃的だった」と評する。小説では39歳だったが、脚本では42歳に直されている。八千草の実年齢は46歳だった[3]。八千草はテレビ大賞主演女優賞を受賞。
また、この作品でデビューした国広富之はゴールデン・アロー賞放送新人賞等を受賞した。

コンセプト
担当ディレクター・島田雄三は今東光の記録小説『悪太郎』を読み、この当時のヤンキー全盛の若者文化との共通点を感じていた[10]。
そんな「普通のことを書かないでほしい」という発注条件のもと[11]、従来の歌謡曲の枠外で曲を作りたいと考えて「反社会」をコンセプトとした売野のある詞案[12]が目に留まったという。

タイトルかつ歌詞の最後に登場する「少女A」は、少年犯罪において実名報道を避けるために用いられる匿名から売野が思いついたもので、中森のイニシャルとは無関係である[12][13][14]。
もともとのタイトルは「少女A（16）」だったが、「少女A（17）」を経て最終的に「少女A」に落ち着いた[12]。

本曲の詞については、もともと沢田研二向けに「ロリータ」のタイトルで提供され廃案になった歌詞（プールサイドで年配の男が10代の少女を口説く設定[12]）を書き直したものだという[9][14]。
また、サビの一部の歌詞も、後述「蒼いシャガールの絵」に元々あった「ねえ、あなた ねえ、あなた」を、「じれったい じれったい」に書き換えたという経緯がある[15]。
これも含め売野は、「プレイバックPart2」（山口百恵）の捨てゼリフなど阿木燿子作品を参考にもしたという[16]。なお島田は最終的に、売野が書いた1番と2番の歌詞を入れ替えた[13]。
